Open Gate is centrally located close to medical clinics, banks and shopping.  The facility is licensed for 16 beds with private rooms and apartments.  We pride ourselves on personal care with dignity, great home cooking and creating an enjoyable comfortable home. At Open Gate Residence, we encourage our residents to achieve their personal goals with comfort and contentment.  We strive to provide quality life services and a caring attitude, so our residents have the opportunity to enjoy the remainder of their lives. We are dedicated to keeping our residents active.  They have the freedom to participate in the in-house activities, as well as, community activities.  The Glenwood Springs community has an excellent organized network of volunteers and community groups, who assist in providing the physical and living accommodations residents need such as senior chat-n-chew, senior traveler, recreation through the Glenwood Springs Senior Center, Church, the Hot Springs Pool, Free Public Concerts, Fair and Holiday Celebrations, libraries, and Adult Education Programs. Open Gate Residence has a prior evaluation to admission for its residents.  The evaluation is shared by staff and family.  The criteria for the residents are that they must have the physical and mental status which does not require medical intervention on a continuing basis.  Upon admission a Care Plan is developed by the new resident, their family, physician and the Open Gate care team.  The Care Plan is the residents assets and skills for independent living, medical and personal needs, as well as, their life style and activities of daily living.  Modifications to the care plan, such as different approaches and requirements, will be made only when the residents needs indicate a change is necessary. Open Gate offers the finest in assisted living for those with progressing limitations brought on by aging.  The management and staff strive to preserve the dignity of every resident, while enhancing their life, health and happiness.
